Urinary incontinence nursing care plan

Urinary incontinence is a condition in which urine leaks regardless of one’s will. It is classified into stress incontinence, overflow incontinence, urge incontinence, reflex incontinence, complete incontinence, and functional incontinence.

5. Urination status (number of times, time, interval, presence or absence of urinary intention, presence or absence of compensatory urinary intention, properties, urine volume, residual urine, feeling of residual urine)
6. Presence or absence of urinary incontinence
7. Drinking time, amount of drinking, content of drink
8. Items used (diapers, pads, etc.)
9. Urination environment (toilet location and type)
10. Urination movement (balance, movement, presence or absence of abdominal pressure)
11. Stress situation

Care plan
1. Consider and encourage drinking water based on urination pattern
2. Consider and encourage urination according to the urination pattern
3. Prepare the urination environment
-Install portable toilets and urinary organs if necessary
-Mark the route to the toilet if necessary
-If possible, adjust to get closer to the toilet
4. Wear clothes that are easy to put on and take off
5. Select the item according to the urination status
6. In case of incontinence (nurse or patient himself) promptly respond to incontinence

2. Explain the importance of knowing the urination pattern
3. Explain that you should urinate early
4. Explain to choose clothes that are easy to put on and take off
